256层螺旋CT在外伤性胸主动脉损伤诊断中的应用价值

摘要

目的 初步探讨256层螺旋CT血管造影对外伤性胸主动脉损伤(ATAI)的应用价值.方法 对8例ATAI患者行胸腹部256层螺旋CT血管造影,并利用原始数据在工作站进行重建处理并分析其影像学特点.结果 8例ATAI患者中,降主动脉破裂2例,主动脉峡部局限性夹层3例,主动脉壁内血肿3例.256层螺旋CT能够清晰显示病变血管征象及解剖关系,如破口的位置、内膜片、真假腔及病变累及的范围等.结论 256层螺旋CT可以进行快速大范围血管成像,多方位、多角度观察受检血管,精确显示ATAI的各种征象,为临床治疗方案的制定提供重要依据.%Objective To evaluate the diagnostic value of 256 slice spiral computed tomography (CT) in acute thoracic aortic injuries (ATAI).Methods Contrast-enhanced was carried out by Philips 256 slice spiral scanner and eight cases of chest trauma patients had continuously strengthen scanning.Volume original data in workstations for reconstruction process and the imaging characteristics were analyzed.Results Eight cases included 2descending rupture,3 aortic hematoma and 3 aortic dissection.All cases were confirmed by clinical surgery.256slice spiral CT could not only display all kinds of ATAI signs clearly,but also showed the other chest traumatic signs:ofsubcutaneous emphysema,fractured ribs,pneumothorax,pleural effusion,etc.Conclusions The 256 slice spiral CT can get large-scale vascular imaging fastly,and display signs of ATAI exactly.It can provide important basis for treatment by direct observation on blood vessels.
