上颌前牙连续牙位缺失种植修复的临床研究

摘要

目的 探讨上颌前牙连续牙位缺失种植修复的美学效果.方法 选取2009年10月至2014年10月于山西省大同市美源口腔医院接受种植治疗的患者12例,均为上颌前牙缺失.术前常规牙周检查和治疗后,在前牙区连续牙位缺失处植入种植体,在种植体愈合期间,制作临时修复体,戴入口内对种植体周围软组织进行诱导塑形,在6个月后取模,制作最终修复体戴入口腔内,3、6、12个月后复查,观察其修复效果,并使用Jemt龈乳头指数进行美学评价.结果 12例患者种植体稳定性良好,与周围组织协调性好,种植体周围骨质致密,无骨吸收,周围软组织健康、色泽正常、附丽高度适中,患者咬合、咀嚼、语音功能良好.Jemt龈乳头指数Ⅱ度及Ⅲ度者11例(91.7％),有1例2枚种植体Jemt龈乳头指数为Ⅰ度.修复体牙间龈乳头高度随时间延长,而呈现增长趋势.结论 采用种植义齿修复连续牙位缺失能满足患者对美观的基本要求.修复体完成后牙间龈乳头高度随时间延长而呈现增长趋势.%Objective To study the aesthetic effect of implant restoration of maxillary anterior tooth on continuous tooth missing.Methods Totally 12 patients with continuous tooth missing of anterior teeth area were enrolled and received dentures implants.During the recovery of implants, the temporary restoration was made and put into the mouth to induce the remodeling of soft tissue around the implants.Six months later, the temporary restoration was taken out and the last restoration was made.The restoration effect was assessed 3,6 and 12 months later with Jemt gingival papilla index.Results In all the patients, the implants were stable and coordinated well with surround tissue;the bones around the implants were dense without bone resorption;the surrounding soft tissue was healthy with normal color and appropriate attachment height;the biting, chewing and phonetic function showed no abnormity.There were 11 cases (91.7％) with Jemt gingival papilla index of grade Ⅱ and Ⅲ,1 case (8.3％) with Jemt gingival papilla index of grade Ⅰ.The implant size increased along with time.Conclusions Dentures implants can meet the artistic requirement in repairing continuous missing tooth.The size of the inter-dental papilla increases along with time.
