多中心合作健康教育模式对提高糖尿病患者自我管理能力的效果观察

             

摘要

目的:探讨有效的健康教育模式,从而提高2型糖尿病(DM)患者自我管理能力和代谢控制。方法对入选的150例2型糖尿病患者采用多中心合作的模式进行健康教育,观察患者糖尿病相关知识的知晓率及自我管理能力等方面的改善情况。结果接受多中心合作模式健康教育的患者自我管理能力总分,饮食管理、药物管理、血糖监测及低血糖管理4方面得分显著提高,差异均有统计学意义(P<0.05);患者空腹血糖(FBG)、餐后2h血糖(2hFBG)、糖化血红蛋白(HbA1C)、体重指数(BMI)较入院时均有改善,差异具有统计学意义(P<0.05)。结论多中心合作健康教育模式可以提高2型糖尿病患者自我管理水平,减少糖尿病的并发症,改善患者的生活质量。%Objective To investigate the effective mode of health education, and improve the self management ability and metabolic control in type 2 diabetes mellitus (DM).Methods150 cases with type 2 diabetes mellitus received health education by polycentric-cooperative mode, the improvement of awareness rate of related knowledge and self-management ability was observed.ResultsThe scores of self-management ability, diet management, drug management, blood glucose monitoring and hypoglycemia management of the patients accepting polycentric-cooperative mode were significantly improved, the differences had statistical significance (P<0.05). The fasting blood glucose (FBG), 2-hour postprandial blood glucose (2hFBG), glycosylated hemoglobin (HbA1c), body mass index (BMI) all were improved ; the differences were statistically significant (P<0.05).Conclusion The polycentric-cooperative mode of health education can increase the self-management ability in patients with type 2 diabetes mellitus, and reduce the complications, and finally improve the quality of their life.
