全腔镜与腔镜辅助小切口技术在甲状腺手术中的效果对比分析

         

摘要

目的:对比分析全腔镜与腔镜辅助小切口技术在甲状腺手术中的效果。方法本研究共包括在我院行甲状腺手术的患者209例,就诊时间为2013年10月~2015年6月,随机分为两组,A组105例,用全腔镜治疗,B组104例,腔镜辅助小切口技术治疗。比较两组术中及术后情况、颈部感觉减退评分及颈前皮肤紧张感评分、术后满意度。结果 A组单侧、双侧手术时间与B组比较明显较长、第1天引流量、引流总量与B组比较较多,引流时间及住院时间与B组比较明显较短,颈部感觉减退评分及颈前皮肤紧张感评分与B组比较均明显较低,术后满意度与B组比较明显较高,两组差异有统计学意义(P<0.05)。结论全腔镜及腔镜辅助小切口技术应用于甲状腺手术均可收到较好的治疗效果,且均有较高的安全性,但也各有不足之处,甲状腺手术患者具体实施的手术方式需综合患者的病情及意愿决定。%Objective To comparatively analyze the effect of total endoscopic and endoscope-assisted small incision technique in thyroidectomy. Methods 209 patients, who were treated with thyroidectomy and were visiting in our hospital from October 2013 to June 2015, were randomly divided into two groups. 105 cases in A group were treated with total laparoscopic, 104 cases in B group were treated with endoscope-assisted small incision technique. To compare the intraoperative and postoperative condition, jugular hypoesthesia score, tensity score of anterior cervical skin, and postoperative satisfaction between the two groups. Results The unilateral and bilateral operation time in A group was obviously longer, the drainage flow of the first day and total drainage flow in A group were more major, the drainage time and hospitalization time in A group were obviously shorter, the jugular hypoesthesia score and the tensity score of anterior cervical skin in A group were obviously lower, the postoperative satisfaction in A group was significantly higher, than which in B group, the differences were statically significant(P<0.05). Conclusion Total endoscopic and endoscope-assisted small incision technique in thyroidectomy could obtain better treatment effect, all has higher safety, but each has deficiencies. It is better to determine the operation methods of implementation in patients with thyroidectomy, according to synthetical patients’disease condition and willingness.
