早期康复治疗脑卒中后肢体运动功能障碍86例

     

摘要

目的:探讨早期康复治疗对脑卒中患者运动功能及日常生活能力的影响.方法:选择2010年1月~2011年1月在我院治疗的86例脑卒中后肢体运动功能障碍患者,将其随机分为康复组和对照组进行临床对照观察,每组各43例.两组患者均给予神经内科常规处理.康复组患者在其神经症状、生命体征平稳后72 h即开始进行康复治疗,采用简式Fugl-Meyer运动功能评定法(FMA)及改良Barthel指数(MBI)测定患者运动功能.结果:治疗前康复组和对照组患者上肢FMA评分[(6.20±1.59)、(6.12±1.42)分]、下肢FMA评分[(6.67±2.21)、(6.31±2.41)分]、MBI评分[(26.15±10.52)、(26.39±10.62)分]均低于治疗后[康复组:(24.62±3.31)、(26.23±7.27)、(34.95±8.26)分,对照组:(11.52±3.63)、(13.84±6.29)、(30.23±7.31)分],差异均有统计学意义(康复组:t=4.965、5.145、3.675,均P<0.05;对照组:t=2.471、2.542、3.274,均P<0.05).治疗后康复组各评分高于对照组(t=2.547、2.965、2.312,均P<0.05).结论:脑卒中的早期康复治疗是有效的,可促进患者功能的恢复,值得在临床上推广应用.%Objective: To explore the effect of early rehabilitation on motor function and activity of daily living of patients with cerebral stroke. Methods: 86 patients with motor disturbance of limb after cerebral stroke treated in our hospital from January 2010 to January 2011 were randomly divided into rehabilitation group (43 cases) and control group (43 cases). Patients in both groups were received neurology conventional treatment; patients in rehabilitation group were also received rehabilitation therapy 72 h after their neurological symptoms and vital signs stable. Fugl-Meyer motor function assessment (FMA) and Modified Barthel index (MBI) were used to evaluate motor function of patients. Results: Before treatment, upper limb FMA scores of rehabilitation group and control group [(6.20±1.59), (6.12±1.42)scores], lower limb [(6.67±2.21),(6.31± 2.41) scores] and MBI scores [(26.15±10.52), (26.39±10.62) scores] respectively were significantly lower than those after treatment [rehabilitation group: (24.62±3.31), (26.23±7.27), (34.95±8.26) scores; control group: (11.52+3.63), (13.84± 6.29), (30.23±7.31) scores] (rehabilitation group: t=4.965, 5.145, 3.675, all P<0.05; control group: t=2.471, 2.542, 3.274, all P<0.05). After treatment, scores of rehabilitation group were significantly higher than those of control group (t=2.547, 2.965, 2.312, all P<0.05). Conclusion: Early rehabilitation is effective for patients with cerebral stroke, it can promote the full restoration of function of patients, and it is worth of clinical application.
