癌胚抗原、糖类抗原125、糖类抗原19-9、糖类抗原125联合检测对胃癌的临床诊断价值研究

摘要

Objective To investigate the clinical diagnosis value of carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125) and carbohydrate antigen 50 (CA50), carbohydrate antigen 19-9 (CA 19-9) four serum tumor markers detection gastric cancer. Methods The serum levels of CEA, CA50, CA19-9, CA125 of 56 patients with gastric cancer and 48 cases of benign stomach group were detected by radioimmunoassay, and compared with 30 normal healthy subjects (healthy control group). Results The serum levels of CEA, CA50, CA19-9, CA125 of gastric cancer patients was significantly higher than the healthy group and the healthy control group preoperative. The serum levels of CEA, CA50, CA19-9, CA125 level of gastric cancer patients postoperative three months significantly reduced than before surgery, the difference was significant (P < 0.05), but the benign gastric disorders group and the healthy control group had no significant difference (P > 0.05). The sensitivity of four kinds joint detection was 87.5%, and was significantly higher than any other single detection, the difference was significant (P < 0.05). The levels of CEA, CA50, CA19-9, CA125 of recurrence group were significantly higher than those without recurrence, the difference were significant (P < 0.05). Conclusion Combined detection of CEA, CA50, CA19-9 and CA125 for gastric cancer diagnosis, treatment and prognosis have important clinical value, and help to improve the sensitivity of the diagnosis of gastric cancer.%目的 探讨癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原50(CA50)、糖类抗原19-9(CA19-9)四种血清肿瘤标记物联合检测对胃癌诊断的临床价值.方法 通过放射免疫分析法测定56例胃癌患者(胃癌组)及48例良性胃病组血清CEA、CA50、CA19-9、CA125水平,并与30例正常健康体检者(健康对照组)作比较.结果 术前胃癌组的CEA、CA50、CA19-9、CA125明显高于良性胃病组及健康对照组,术后3个月胃癌组患者血清CEA、CA50、CA19-9、CA125水平较术前明显降低,差异有统计学意义(P<0.05),良性胃病组与健康对照组比较,差异无统计学意义(P>0.05).四项联合检测的灵敏度达87.5%,明显高于其他任一单项检测的灵敏度,差异有统计学意义(P<0.05).复发者CEA、CA50、CA19-9、CA125的水平明显高于未复发者,差异有统计学意义(P<0.05).结论 联合检测血清CEA、CA50、CA-9-9、CA125对胃癌的诊断、治疗及预后等方面具有重要的临床价值,有助于提高胃癌诊断的敏感性.
