急性冠脉综合征患者超敏C 反应蛋白变化及与冠脉病变程度的关系

     

摘要

目的 探讨超敏C 反应蛋白(hs-CRP)在急性冠脉综合征(ACS)患者中的差别,并探讨其与ACS 患者冠脉病变严重程度的关系及其临床意义.方法 序贯入选住院并接受冠脉造影ACS 患者共105例,按临床诊断将ACS 患者分为三组,再根据Gensini 积分将其再分为三组进行比较分析.另设稳定性心绞痛(SA)组和正常冠脉组,将其作为对照组.入院即刻抽取患者静脉血进行hs-CRP 检查.结果 ①ACS 患者(包括不同临床类型亚组)的hs-CRP 水平均高于SA 组和正常冠脉组,差异具有统计学意义(P < 0.05).②hs-CRP 水平在不同冠脉病变程度ACS 患者中存在差异(P < 0.05)且hs-CRP 水平与反映冠脉病变程度的Gensini 积分存在正相关(r = 0.741,P < 0.05 ).结论 ACS 患者的hs-CRP 水平显著高于SA 组及正常冠脉组水平,且与冠脉病变的严重程度有关,hs-CRP 可作为临床上评价入院ACS 患者危险程度的简单、有效、实用的方法.%Objective To study the diversity of levels of high-sensitivity C-reactive protein (hs -CRP) in patients with acute coronary syndrome (ACS), and to investigate the relationships between the biochemical markers and the extent of coronary artery lesions of ACS, and to assess the clinical value. Methods A total of 105 cases of ACS admitted were divided into three clinical types. These patients were also evaluated by Gensini score for a decision of coronary angiography (CAG) after admission, and divided into three groups. Stable angina patients and normal volunteers were designated as control group. The venous blood samples were taken from the patients immediately after admission for detection of hs-CRP. Results ①Significant differences in hs-CRP were found between ACS group including the different clinical subgroups and SA group/ normal group (P < 0.05). ②Significant differences in hs-CRP were found among three ACS groups which were divided according to the severity of coronary artery disease in patients (P < 0.05). There was a positive correlation between levels of hs-CRP and Gensini score in ACS patients (r = 0.741, P < 0.05). Conclusion The levels of hs-CRP in patients of ACS are much higher than those in SA group/ normal group. There is a correlation between levels of hs-CRP and different extent of coronary artery lesions in ACS patients. The hs-CRP can be helpful and effective for risk stratification of the hospitalized patients with ACS after admission.
