目的 探讨垂体腺瘤超微结构电镜诊断的意义.方法 回顾性分析2010年8月~2015年8月首都医科大学附属北京天坛医院神经外科垂体腺瘤电镜切片的标本112例,对比分析超微结构电镜结果与按传统分类的异同.结果 本组112例垂体腺瘤样本,47例传统标准诊断的无功能垂体腺瘤(电镜结果修正为:20例无功能垂体腺瘤,21例垂体瘤样细胞瘤,3例垂体干细胞瘤,2例促性腺激素腺瘤,1例混合性垂体瘤),65例功能性垂体腺瘤(电镜结果:泌乳素腺瘤10例,生长激素腺瘤15例,促甲状腺激素腺瘤2例,促肾上腺皮质激素腺瘤5例,促性腺激素腺瘤2例,垂体干细胞瘤1例及混合性垂体瘤30例).结论 超微结构电镜在垂体腺瘤的诊断中,能够提供更多的生物学信息,在临床中有比较重要的意义.%Objective To investigate the significance and perspective of ultrastructural characteristics in diagnosis of pituitary adenoma.Methods From August 2010 to August 2015,the specimens of 112 patients with pituitary adenoma at the Department of Neurosurgery,Beijing Tiantan Hospital were analyzed retrospectively.Making a comparison of the routine pathology approaches and electron microscope in diagnosis of pituitary adenoma.Results In 112 pituitary adenoma species,47 tumors met the routine criterion of clinically non-functioning adenoma.Within those tumors,20 were diagnosed as clinically nonfunctioning adenoma by electron microscope,the others including 21 oncocytoma subtype of non-functioning pituitary adenomas,3 stem cell adenomas,2 gonadotropinsadenomas and 1 mixed type adenoma.Besides,65 tumors met the routine criterion of functional pituitary adenoma,including 10 prolactinomas,15 growth hormoneproducing adenomas,5 ACTH-secreting adenomas,2 gonadotropins adenomas,2 TSH-secreting adenomas,1 stem cell adenoma and 30 mixed type adenomas by electron microscope.Conclusion Evaluation of ultrastructural characteristics evaluating of pituitary adenomas can provide extraordinary information,which has contributed considerably to the diagnosis of pituitary adenomas.
