应用脉搏波传导速度评估高脂血症患者颈动脉弹性的意义

         

摘要

imaging evaluating carotid artery elasticity in patients with hyperlipidemia.Methods: A total of 304 persons were investigated, including 203 patients with hyperlipidemia and 101 healthy persons, excluding high blood pressure, diabetes, cerebrovascular disease, coronary heart disease, smoking, carotid artery plaque and other factors. PWV at two time points were detected, the beginning of systolic(BS) and the ending of systolic(ES), applying ultrafast imaging technology of Aixplorer ultrasound scanner. The value of BS and ES were compared between two groups, and the correlations were tested between BS and ES and four blood lipid indices, including total cholesterol (CHOL), triglyceride (TG), high density lipoprotein (HDL-C) and low-density lipoprotein(LDL-C).Results: The value of BS was higher in hyperlipidemia group (5.74±1.24m/s) than in healthy control group (5.42±1.13m/s), and the difference has statistic significance(F=0.867, P<0.05). The value of ES was higher in hyperlipidemia group (7.94±2.04m/s) than in healthy control group (7.07±2.03m/s), and the difference has statistic significance (F=0.011,P<0.05). Correlation test showed BS positively correlated with TG, but there was not correlation between BS and CHOL, HDL-C, LDL-C. ES and CHOL, TG, HDL-C, LDL-C were correlated. ES and CHOL, TG, LDL-C was positively correlated, and with HDL-C negative.Conclusion: PWV based on ultrafast imaging technology can reflect the carotid atherosclerosis in patients with hyperlipidemia well, and ES is more sensitive in reflecting changes in carotid artery elasticity than BS.%目的:探讨基于超高速成像技术的脉搏波传导速度(PWV)评价高脂血症患者颈动脉弹性变化的意义。方法:选取在医院体检中心体检的304名体检者,根据高脂血症临床的诊断标准,将其分为高脂血症组(203例)和健康组(101名),排除高血压、糖尿病、脑血管疾病、冠心病、吸烟以及颈动脉斑块等因素。应用Aixplorer超声诊断仪的超高速成像技术,检测收缩期起始的PWV(BS)和收缩期末的PWV(ES)。对比两组的BS和ES,并检测BS和ES与总胆固醇(CHOL)、甘油三酯(TG)、高密度脂蛋白(HDL-C)以及低密度脂蛋白(LDL-C)的相关性。结果:两组BS值相比,高脂血症组(5.74±1.24)m/s高于健康组(5.42±1.13)m/s,差异有统计学意义(F=0.867,P<0.05);ES值相比,高脂血症组(7.94±2.04)m/s高于健康组(7.07±2.03)m/s,差异有统计学意义(F=0.011, P<0.05)。相关性检验结果显示,BS与TG具有正相关性,与CHOL、HDL-C及LDL-C均无相关性,ES与CHOL、TG、HDL-C以及LDL-C均有相关性,其中ES与CHOL、TG及LDL-C呈正相关,与HDL-C呈负相关。结论:基于超高速成像技术的PWV能够较好地反映高脂血症患者颈动脉硬化,且在反映颈动脉弹性变化上ES较BS更加敏感。
