低频电刺激在宫颈癌术后难治性尿潴留预防中的应用

摘要

目的:探讨低频电刺激在宫颈癌术后难治性尿潴留预防中的应用.方法:选取医院收治的200例宫颈癌患者,依据护理方法将采用尿管护理的患者纳入传统护理组,采用尿管护理联合低频电刺激护理的患者纳入联合护理组,每组100例.统计分析两组患者的尿潴留发生情况和重置导尿管情况,对两组患者的残余尿量、留置尿管时间进行观察和记录,并对其护理满意度进行评分,运用手动方法对两组患者的盆底肌力进行测定.结果:联合护理组患者的尿潴留发生率和重置导尿管率分别为11.0%和6.0%,均显著低于传统护理组的50.0%和19.0%,差异均有统计学意义(x2=14.45,x2=12.83;P＜0.05);残余尿量显著少于传统护理组,留置尿管时间显著短于传统护理组,护理满意度评分显著高于传统护理组,差异有统计学意义(t=4.303,t=2.776,t=3.182;P＜0.05);肌力分级0级、Ⅰ级和Ⅱ级比例均显著低于传统护理组,差异均有统计学意义(x2=13.28,x2=14.41,x2=5.02;P＜0.05),Ⅲ级、Ⅳ级和Ⅴ级比例均显著高于传统护理组,差异均有统计学意义(x2=11.14,x2=9.35,x2=7.38;P＜0.05).结论:低频电刺激在宫颈癌术后难治性尿潴留预防中的应用效果良好.%Objective: To investigate the application of low frequency electrical stimulation in preventing refractory urinary retention post cervical cancer surgery. Methods: According to different nursing method, 200 patients with cervical cancer were divided into conventional nursing group(100 cases) who received ureter nursing and combining nursing group(100 cases) who received ureter nursing combined with low frequency electrical stimulation. The situations of urinary retention and replacement of catheter were analyzed as statistical method, and the residual urine volume, time of indwelling catheter of the two groups were observed and recorded, and the degree of satisfaction about nursing method was graded. Besides, the manual operation was applied to measure myodynamia of pelvic floor of each patients. Results: The occurrence rate of urinary retention and replacement rate of catheter of combining nursing group(11.0% and 6.0%) were significantly higher than that of conventional nursing group(50.0% and 19.0%), respectively (x2=14.45, x2=12.83, P<0.05). And the residual urine volume, time of indwelling catheter and degree of satisfaction of combining nursing group were significantly better than that of conventional group(t=4.303, t=2.776, t=3.182, P<0.05), respectively. Besides, the proportions of 0 grade, I grade and II grade of myodynamia grade of combining nursing group were significantly lower than that of conventional nursing group(x2=13.28, x2=14.41, x2=5.02, P<0.05), respectively, while the proportions of III grade, IV grade and V grade of combining nursing group were significantly higher than that of conventional nursing group(x2=11.14, x2=9.35, x2=7.38, P<0.05). Conclusion:The application effect of low frequency electrical stimulation is well in preventing refractory urinary retention post cervical cancer surgery.
