64排螺旋CT脑成像在诊断脑血管畸形中的应用价值

         

摘要

目的 探讨64排螺旋CT血管成像对脑血管畸形的诊断价值.方法 利用64排螺旋CT对20例脑动静脉畸形、2例脑静脉畸形、2例海绵状血管瘤患者行CT血管成像检查.对扫描图像分别进行多平面重组、容积再现及最大密度投影.结果 CT血管造影术(CTA)能显示脑动静脉畸形(AVM)的病变形态及准确部位,有效显示供血动脉、引流静脉及血管巢.能清晰显示脑静脉畸形静脉血管的组成及引流静脉的数目和方向.但不能显示海绵状血管瘤的血管结构,对其诊断价值不大.结论 64排螺旋CT血管成像能清晰显示具有较粗大供血动脉或引流静脉、血管巢的脑血管畸形,对脑血管畸形的诊断及治疗具有重要的临床价值.%Objective To explore the value of 64-slice spiral CTA(CT Angiography) in diagnosis of cerebral vascular malformations. Methods Twenty cases of cerebral AVM(Arteriovenous Malformations), two cases of cerebral venous malformations and two cases of cavernous hemangiomas patients were scanned with 64-slice spiral CTA. Then, VR(Volume Rendering), MPR(Multiplanar Reformation) and MIP(Maximum Intensity Projection) were performed respectively for the scanned images. Results Through application of CTA, the lesion morphology and exact location of the cerebral AVM were visualized with good display of blood-supply arteries, draining veins and vascular nest; as for cerebral venous malformations, the composition of veins as well as the number and the direction of draining veins was clearly shown. However, CTA failed to make the vascular structure of cavernous hemangiomas displayed and was of little value in diagnosis of cavernous hemangiomas. Conclusion The 64-slice spiral CT angiography could make cerebral vascular malformations with thick blood-supply arteries or draining veins and vascular nest clearly revealed, which was of great clinical value in diagnosis and treatment of cerebral vascular malformations.
