X线断层融合摄影在Halo架固定结合枕颈融合治疗寰枢关节脱位术后评价中的价值

         

摘要

目的:讨论X线断层融合摄影(DTS)在Halo架固定结合枕颈融合治疗寰枢关节脱位术后的应用价值。方法收集Halo架固定结合枕颈融合治疗寰枢关节脱位患者58例,分别行张口位、颅颈交界侧位及以寰枢关节为中心的颅颈交界区正、侧位断层融合摄影。分析两种方法的图像质量及诊断结果。结果DTS优秀图像为54例(93.1%)、良好4例(6.9%)、差0例,DTS诊断寰枢关节复位成功52例,无法评价2例。常规DR优秀图像28例(48.3%)、良好17例(29.3%)、差13例(22.4%),DR诊断寰枢关节复位成功40例,无法评价18例。经统计学分析,两种检查方法获得的图像质量及诊断率差异具有统计学意义(P<0.05)。结论DTS能很好显示Halo架固定下枕颈融合术后的寰枢关节形态及结构,可作为寰枢关节复位术后效果评价及随访的首选检查方式。%Objective To discussed the value of the digital tomosynthesis in the evaluation of atlantoaxial dislocation treated with occipitocervical fusion and Halo frame. Methods Both DR and digital tomosynthesis scanning were performed in 58 cases of atlantoaxial dislocation. Two kinds of imaging were analyzed with regard to image quality and atlantoaxial shape. Results In 58 cases,image quality of atlantoaxial was better with digital tomosynthesis(excel ent image rate:93.1%) than with DR(excel ent image:48.3%). 52 cases with reconstruction of atlantoaxial dislocation were found by digital tomosynthesis scanning,DR only found 40 cases. Conclusion X-ray digital tomosynthesis technique can improve image quality of atlantoaxial dislocation treated with occipitocervical fusion and Halo flame. It is expected to become the first method to evaluating the atlantoaxial treated with occipitocervical fusion and Halo flame.
