脑发育性静脉异常在高、低场强磁共振中的不同影像学特点

         

摘要

目的:探讨脑发育性静脉异常(DVA)在高、低场强磁共振的不同影像学特点,增加对病变差异化表现的认知。方法回顾性分析我院6例低场强磁共振、21例高场强磁共振平扫及增强表现,研究其引流静脉及髓静脉的信号特点。结果6例低场强磁共振平扫有3例异常粗大的引流静脉得以显示,其在T1WI上均为低信号,在T2WI上1例为低信号,2例为高信号,髓静脉未有显示。增强扫描6例都显示了引流静脉和髓静脉,其中2例呈典型的“水母头”征,另外4例分别显示了引流静脉和邻近的髓静脉;21例高场强磁共振平扫较粗大的引流静脉均得以显示,T1WI呈高信号3例,等信号4例,低信号14例,T2WI呈高信号9例,低信号12例。髓静脉显示16例,呈典型“水母头”征11例, T2WI均为高信号。结论在高、低场强磁共振上DVA有着不同的影像学特点:低场强磁共振需要增强扫描才能明确诊断;高场强磁共振表现更具有典型性,绝大部分病例在平扫的基础上就能确立诊断,而且检查序列多样化。%Objective To investigate the imaging characteristics of developmental venous abnormality in high-field and low-field MRI,to improve our recognition about this disease difference performance. Methods Six cases in low-field MRI and twenty-one cases in high-field MRI were retrospectively analyzed the DVA features by scan and enhance MRI,an evaluation the displaying signal characteristics of draining veins and medul ary veins. Results There were three large draining veins cases can be shown,two cases were the low signal on T1WI,and one case of low signal and two for the high signal on T2WI, it fails to show the medul ary veins. Six cases post-contrasted MRI displayed the markedly enhanced draining veins and medul ary veins,which two cases showed the typical sign of“medusa head”,and four cases showed a draining vein and the adjacent medul ary veins. Twenty-one large draining veins cases can be shown,three cases were the high signal on T1WI,four cases were the equisignal,fourteen cases were the low signal,nine for the high signal on T2WI,twelve for the low signal. Sixteen were show the medul ary veins,eleven cases showed the typical sign of“medusa head”,T2WI were al showed high signal. Conclusion There are different imaging characteristics of developmental venous abnormality in High-field and low-field MRI,low field MRI need enhanced scan can clear diagnosis;high field MRI manifestations are more typical,great majority of cases in plain scan can establish the diagnosis,and check the sequence were diversity.
