Objective To investigate the effect of vacuum sealing drainage combined with muscle flap transplantation in the treatment of open fractures of the tibia complicated with chronic osteomyelitis. Methods Chose 100 cases of patients with fracture complicated with chronic osteomyelitis from 2013 June to 2014 June in our hospital,randomly divided into observation group and control group,50 cases in each group. The control group underwent conventional treatment,the observation group patients for vacuum sealing drainage combined with muscle flap transplantation therapy,compared two groups of patients with treatment effect. Results The therapeutic effect of patients in observation group was significantly better than the control group, the cure rate and satisfaction rate in significantly was higher than control group,and the infection rate and joint stiffness was significantly lower than the control group(P<0.05). Conclusion Vacuum sealing drainage combined with muscle flap transposition for treatment of open fractures of the tibia chronic osteomyelitis complicated has significant clinical effect and less complications.%目的：探讨负压封闭引流术联合肌皮瓣移植治疗胫骨开放性骨折并发慢性骨髓炎的临床效果。方法选取2013年6月～2014年6月期间在我院接受治疗的胫骨开放性骨折并发慢性骨髓炎患者100例，随机将其分为观察组与对照组，每组50例。对照组患者行传统治疗；观察组患者行负压封闭引流术联合肌皮瓣移植治疗，比较两组患者的治疗效果。结果观察组患者的治疗效果优于对照组，其在治愈率与满意率高于对照组，而感染率与关节僵硬率低于对照组（P<0.05）。结论负压封闭引流术结合肌皮瓣转位治疗胫骨开放性骨折并发慢性骨髓炎临床效果显著，并发症较少，可加速患者康复。
