HCG调控期血清雌激素水平变化对体外受精-胚胎移植妊娠结局及子代安全的影响

         

摘要

目的:探讨体外受精-胚胎移植新鲜周期中注射HCG日及注射HCG后2 d(取卵日)血清雌二醇(estradiol,E2)水平的变化对妊娠结局及子代安全的影响。方法选择2013年1月~2015年2月在我中心行第一周期体外受精-胚胎移植助孕患者1590例,回顾性分析所有患者注射HCG日及注射HCG后2 d血清E2水平,统计注射HCG日与注射HCG后2 d血清E2比值,探讨HCG调控期血清E2水平变化对妊娠结局及子代安全的影响。结果水平组HCG日血清E2水平高于下降组及上升组,下降组取卵日血清E2水平低于水平组及上升组,差异有统计学意义(P<0.05)。上升组临床妊娠率、种植率及出生率高于下降组及水平组,差异有统计学意义(P<0.05)。在子代安全方面,3组患者差异无统计学意义。结论HCG调控期E2水平变化可预测妊娠结局,但对子代安全无影响。%Objective To explor the effect of the change serum E2 level on HCG day and on two days after HCG day on the outcome and baby safety of in vitro fertilization-embryo transfer.MethodsWe chose 1 590 patients who all underwent first in vitro fertilization-embryo transfer between January 2013 and February 2015 and retrospectively analyzed serum E2 level on HCG day and two days after HCG and their ratio, explore the effect of the change serum E2 level on HCG day and on two days after HCG day on the outcome and baby safety.ResultsThe serum E2 level on HCG day were significantly higher in plateau group than in decrease group and in increase group. The serum E2 level on two day after HCG were significantly lower in decrease group than in plateau and increase group(P<0.05). The clinical pregnancy rate and implantation rate and live birth rate were significantly higher in increase group than in decrease group and in plateau group(P<0.05). But there were no significant differences in baby safety.Conclusion The serum E2 level change on HCG day and two day after HCG day can predict pregnancy outcome,but had no obvious effect on baby safety.
