多普勒超声监测子宫动脉血流在妊娠期高血压疾病管理中的价值研究

摘要

Objective Analysis and Study on the value of Doppler ultrasound in the management of uterine arterial blood flow during pregnancy. Methods Randomly selected from May 2013 to May 2015 in the hospital obstetrics and Gynecology Department of 50 cases of gestational hypertension in pregnant women as the observation group, elected in the same period in our hospital to accept the inspection of normal pregnant women as control group, two groups were treated with Doppler ultrasound monitoring of uterine artery blood flow, contrast two groups of pregnant women the uterine artery hemodynamics and pregnancy outcome. Results The observation group Doppler ultrasound examination showed a systolic blood flow spec-trum curve steep, slow down. In control group, the blood flow spectrum of pregnant women was low resistance type, and the test results were within normal range. Observation group pregnant women PI, RI and S / D, diastolic blood early notch oc-currence probability compared with the control group had significant difference (P<0.05). The neonatal weight, neonatal as-phyxia, premature delivery and fetal distress in the observation group were significantly different from the control group (P<0.05). Conclusion Using Doppler ultrasound in the management of hypertensive disorder complicating pregnancy monitoring uterine artery blood flow can be found in the uterine artery early diastolic notch situation to predict all kinds of complica-tions,to provide the necessary basis for the clinical diagnosis and treatment,it is worth to popularize in the clinical applica-tion.%目的 分析研究多普勒超声监测子宫动脉血流在妊娠期高血压疾病管理中的价值. 方法 随机选取2013年5月—2015年5月该院妇产科接诊的50例妊娠期高血压孕妇为观察组,另选同时期于该院接受检查的正常妊娠孕妇作为对照组,两组均采用多普勒超声监测子宫动脉血流,对比两组孕妇各项子宫动脉血流动力学指标及妊娠结局.结果 观察组孕妇多普勒超声血流频谱检查结果显示收缩期曲线上升陡直,下降缓慢.对照组孕妇血流频谱则表现为低阻型,各项指标检测结果均在正常范围内. 观察组孕妇PI、RI、S/D、舒张早期切迹发生几率等与对照组相比均差异有统计学意义(P<0.05). 观察组新生儿体重、新生儿窒息率、早产率及胎儿窘迫发生几率等与对照组相比均差异有统计学意义(P<0.05). 结论 采用多普勒超声在妊娠期高血压疾病管理中监测子宫动脉血流可发现子宫动脉舒张早期切迹情况,预测各类并发症的发生情况,为临床诊疗提供必要依据,值得在临床推广应用.
