目的：观察抗菌药物专项点评对医院抗菌药物合理用药指标的影响。方法通过定期开展专项点评,将点评模式优化前(2013年)、优化后(2014、2015年)的点评结果即合理用药指标进行比较和统计学处理,计数资料采用χ2检验。结果处方指标中门诊患者抗菌药物使用率、急诊患者抗菌药物使用率始终达标,门急诊抗菌药物处方合理率未达标但2015年与2014年相比明显上升,差异有统计学意义(P﹤0.05)。抗菌药物用药指标中的使用率、使用强度、微生物送检率均达标；住院患者抗菌药物使用率、住院患者抗菌药物使用合理率2015年与2013年、2014年相比明显上升,差异有统计学意义(P﹤0.01；P﹤0.05)；特殊使用级抗菌药物使用合理率2015年与2014年相比上升,差异有统计学意义(P﹤0.05)。外科清洁手术预防用药指标中：清洁切口手术预防使用率达标,2015年与2013年、2014年相比呈现明显下降,差异有统计学意义(P﹤0.01)；术前0.5~2 h给药合理率、品种选择合理率2015年与2013年相比呈现上升,差异有统计学意义(P﹤0.01)；术前0.5~2 h给药合理率与2014年相比,差异有统计学意义(P﹤0.05)。结论持续的抗菌药物专项处方点评的开展,使得医院合理用药水平明显提升并持续改进。%Objective To observe the effect of antibiotics antibacterial drugs special comments on hospital drug use indica-tors. Methods By regularly carry out special comment, comment on mode before optimization (2013), an optimized after (2014, 2015) reviews the indicators of rational drug use were compared and statistically, count data using chi square test. Results Prescription indicators of outpatient antibiotic use rate, patients presenting to the emergency department with the usage of antibacterial drugs always compliance, outpatient and emergency prescriptions for antibiotics reasonable rate did not meet but in 2015 and 2014 significantly increased, and the difference was statistically significant (P< 0.05). Antibacte-rial index of drug use rate and intensity of use, microbial inspection rate are standard; inpatient antimicrobial usage, inpa-tient antimicrobial rate of rational use in 2015 and in 2013 and 2014 compared increased significantly, the difference is statistically significant (P<0.01;P<0.05);special use level of antibacterial drugs reasonable utilization in 2015 and 2014 compared to the rise, the difference is statistically significant (P<0.05). Clean surgical operation to prevent drug use indi-cators: clean incision prevention using the up to standard rate, 2015 and 2013, 2014 compared has decreased significantly, the difference was statistically significant (P< 0.01); preoperative 0.5-2 administration reasonable rate, varieties to choose reasonable rate in 2015 and 2013, compared to upward, the difference was statistically significant (P< 0.01); preoperative 0.5-2 administration reasonable rate compared with the 2014, the difference is statistically significant (P<0.05). Conclu-sion The lasting antibacterial drugs special prescription to carry out, the level of rational use of drugs in hospital improved significantly and continuous improvement.
