环状软骨舌骨会厌固定术治疗声门型喉癌的效果研究

摘要

Objective To analyze and discuss the therapeutic effect of cricoid cartilage hyoepiglottic fixation in the treatment of glottic laryngeal carcinoma. Methods 20 patients with glottic laryngeal carcinoma treated in our hospital from June 2013 to June 2014 were treated with cricoid cartilage hyoepiglottic fixation. The situation of the patients after treatment was investigated and compared with that before treatment. Results There were statistically significant differences between the symptoms, such as hoarseness, difficulty in breathing, pharyngeal foreign body sensation, dry cough and sputum with blood status, after treatment and that before treatment, P<0.05. The distance from margin of mucosal resection to tumor edge in all the patients was more than 0.5cm, and margin of mucosal resection was all negative by section examination, with the negative rate of 100%. All the patients with mucosal resection margin from the tumor edge distance exceeds 0.5cm, the cut edge of frozen section examination was nega-tive, the negative rate of 100%. In 20 patients there were 12 with significant curative effect, accounting for 60%;and 6 with effect, accounting for 30%; and 2 without effect, accounting for 10%. After treatment, in the 20 patients, larynx fistula occurred in 2 pa-tients, hematoma of pharynx in 4 patients, and the symptoms of all the patients after symptomatic treatment were improved. After follow-up of half a year, 2 patients died of recurrent disease and invalid treatment. No recurrence of the disease and tumor metas-tasis was found in the remaining patients. Conclusion The therapeutic effect of cricoid cartilage hyoepiglottic fixation in the treat-ment of glottic laryngeal carcinoma is remarkable, and with high safety it is an effective treatment method.%目的：对环状软骨舌骨会厌固定术在声门型喉癌中的治疗效果进行分析讨论。方法随机选择该院2013年6月—2014年6月间收治的20例声门型喉癌患者，所有患者均采用环状软骨舌骨会厌固定术治疗，对患者治疗治疗后的情况进行调查，并比较患者治疗前后状况。结果治疗前声音嘶哑、呼吸困难、咽部异物感、干咳、痰中带血状况与治疗后比较存在明显差异（P＜0.05）。所有患者粘膜切缘距离肿瘤边缘距离均超过0.5 cm，切缘冰冻检查均为阴性，阴性率100%。20例患者治疗效果显著的有12例，占比例的60%；治疗有效的6例，占比例的30%；治疗无效2例，占比例的10%。治疗后20例患者中有2例咽喉瘘道、4例咽部血肿病例发生，所有患者经过对症治疗后症状均得以改善。随访后半年有2例患者疾病复发，后治疗无效死亡，其余患者未发现疾病复发、肿瘤转移情况。结论环状软骨舌骨会厌固定术在声门型喉癌中具有较为显著的疗效，且安全性非常高，是一种安全有效的治疗方式。
