Objective To investigate the curative effect and complications of intra-articular calcaneal fracture. Methods Convenient selected August 2013 to 2014 August in the surgical treatment of intra-articular calcaneal fracture, a total of 120 cases, according to the operation group, anatomy of titanium plate internal fixation for the observation group a total of 63 cases of improved lateral incision, shoe assisted percutaneous pinning prying dial fixed as the control group, a total of 57 patients, Gu of two groups of operation time, fracture healing time, Bohler angle and gissane angle, Maryland score to evalu-ate clinical efficacy. Results Open reduction anatomical titanium plate internal fixation group, the operation time (59.3 ± 9.6) min, fracture healing time (16.5 ± 1.8) week, Bohler angle (25.0±2.7) gissane angle (127.3±3.8) degree. The Maryland foot score (54.5 ± 3.7)score. Shoe assisted with percutaneous pinning fixation group operation time(44.6 ± 6.3) min, fracture healing time(12.74 ± 1.2) weeks and Bohler angle (22.2 ± 3.6) degrees and gissane angle(121.2± 5.7) degree. The Maryland foot score (37.2 ± 4.2) score, the excellent and good rate was compared with open reduction anatomical titanium plate inter-nal fixation group advantage is obvious, with statistical significance (P<0.05). Conclusion The use of the lateral calcaneal modified incision, anatomical titanium plate internal fixation of intra-articular calcaneal fracture effect is ideal, it is worthy of clinical application.%目的:探讨跟骨关节内骨折的手术疗效及并发症。方法方便选取2013年8月一2014年8月在该院手术治疗的跟骨关节内骨折共120例,根据手术方式分组,外侧改良切口解剖型钛板内固定为观察组共63例,靴型支具辅助经皮穿针撬拨固定为对照组共57例,回顾性研究两组的的手术时间,骨折愈合时间, Bohler 角,Gissane角,Maryland评分等,以评价临床疗效。结果切开复位解剖钛板内固定组的手术时间为(59.3±9.6)min,骨折愈合时间(16.5±1.8)周, Bohler角(25.0±2.7)°,Gissane角(127.3±3.8)°,Maryland评分(54.5±3.7)分。靴型支具辅助经皮穿针撬拨固定组的手术时间为(44.6±6.3)min,骨折愈合时间(12.74±1.2)周, Bohler 角(22.2±3.6)°,Gissane 角(121.2±5.7)°,Maryland 评分(37.2±4.2)分。Maryland评分、优良率比较,切开复位解剖钛板内固定组优势明显,差异具有统计学意义(P<0.05)。结论采用跟骨外侧改良切口,解剖型钛板内固定跟骨关节内骨折效果理想,值得临床推广。
