经阴道彩色多普勒超声结合血β-HCG值对宫外孕早期诊断

摘要

目的：探讨经阴道彩色多普勒超声结合血β-人绒毛膜促性腺激素(β-HCG)值对宫外孕早期诊断的临床价值。方法整体选取尿β-HCG检查阳性,高度怀疑为早期异位妊娠的患者106例,均为该院2012年1月—2015年10月收治的病例。对患者进一步进行血β-HCG定量检测,可分为：血β-HCG<200 mIU/mL者15例,血β-HCG 200~1000 mIU/mL者43例,血β-HCG>1000 mIU/mL者48例。对患者进行血β-HCG定量检测,经腹部超声(TAS)以及经阴道彩色多普勒超声(TVCDS)检查。结果经TAS检查,初次确诊宫外孕患者87例,诊断符合率为82.08%经TVCDS检查,初次确诊宫外孕患者102例,诊断符合率为96.23%,TAS与TVCDS诊断符合率差异有统计学意义(P<0.05)；血β-HCG监测结合TVCDS对4例待诊患者进行复诊,确定宫外孕3例,宫内早孕1例,诊断符合率为100%。结论 TVCDS联合血β-HCG值监测,能够有效提高早期宫外孕的临床诊断准确率,可以作为早期诊断宫外孕的首选方法。%Objective To study the clinical value of color Doppler ultrasound and blood β-human chorionic gonadotropin (β-HCG) value in early diagnosis of ectopic pregnancy. Methods 106 cases of patients highly suspected with early ectopic pregnancy due to positive urine β-HCG examination result from January 2012 to October 2015 were selected and divided into the 15 cases whose bloodβ-HCG<200 mIU/mL, 43 cases whose bloodβ-HCG were 200~1 000 mIU/mL and 48 cas-es whose blood β-HCG >1 000 mIU/mL, and the patients were given blood β-HCG quantitative detection, transabdominal ultrasound examination and transvaginal color Doppler ultrasound examination. Results After TAS, 87 cases were initially diagnosed with ectopic pregnancy, and the diagnostic accordance rate was 82.08%, after TVCDS, 102 cases were initially diagnosed with ectopic pregnancy, and the diagnostic accordance rate was 96.23%, and the difference in the diagnostic ac-cordance rate of TAS and TVCDS was obvious (P<0.05);4 cases were followed up by blood β-HCG detection and TVCDS, and 3 cases were diagnosed with ectopic pregnancy, 1 case was diagnosed with intrauterine early pregnancy and the diag-nostic accordance rate was 100%. Conclusion TVCDS and blood β-HCG detection can effectively improve the clinical di-agnostic accurate rate of early ectopic pregnancy, which can be used as the preferred method of early diagnosis of ectopic pregnancy.
