2011年广西3县HIV/AIDS死亡原因回顾性调查

摘要

Objective To explore the causes of death of HIV/AIDS in Guangxi, and scientifically determine the constitution of the causes. Methods A retrospective survey was performed on the demographic characteristics, underlying causes of death and mortality among 277 death cases of 3 counties in Guangxi. Results Among the 277 death cases, 84.48%were male, and 15.52%were female. The average age of these death cases was 53 years. The underlying cause of death relevant to HIV/AIDS accounted for 78.34%, while the proportion of non-AIDS related death was 15.16%. The cause of death for the rest cases of 6.50% could not be determined. Among these death cases, only 58.48%had CD4 test. Among the 40 death cases, who received HAART, 18(45.00%) patients’ CD4 was less than 50/μL;For 122 death cases, who had CD4 test but HAART naïve, 70 (57.38%) patients’CD4 was less than 50/μL, the estimated mortality was 8.98%.Conclusion Relatively high mortality of HIV/AIDS is definitely associated with late HAART treatment or even HAART naive. Early diagnosis and early treatment are more effective approaches of reducing the HIV incidence and mortality of HIV/AIDS.%　　目的探索广西艾滋病病毒感染者（HIV）和艾滋病病人（AIDS）死亡的原因，科学阐明艾滋病病毒感染者和病人的死因构成。方法采用回顾性调查分析广西3个县2011年1~7月HIV/AIDS死亡病例中，人口学特征、死因构成和病死率等情况。结果在277例死亡的HIV/AIDS中，男性占84.48%，女性占15.52%；平均年龄为53岁；死因属于与艾滋病疾病相关症状死亡占78.34%，其他疾病死亡占15.16%，无法判断死因占6.50%；CD4检测率为58.48%；接受抗病毒治疗的40例死亡个案中CD4＜50个/µL有18例（占45.00%），未接受抗病毒治疗且做过CD4检测的122例个案中， CD4＜50个/µL的有70例（占57.38%），病死率为8.98%。结论艾滋病病毒感染者和病人的高死亡率与较晚接受抗病毒治疗甚至不接受抗病毒治疗有密切关系。早发现、早治疗是未来降低艾滋病新发感染率和死亡率最重要的手段。
