IL-23R基因在强直性脊柱炎患者外周血中的表达研究

摘要

目的 研究强直性脊柱炎(ankylosing spondylitis,AS)中IL-23R的表达和调节作用.方法 分别抽取强直性脊柱炎患者(n=61)和正常对照者(n=40)外周静脉血1 ml,以Trizol(invitrogen)法抽提总RNA,并逆转录为cDNA,通过实时荧光定量聚合酶链反应法分析其IL-23R的表达水平,实验数据以内参基因标化.以Bath强直性脊柱炎活动性指标(BASDAI)作为病情评估标准,;≤4分为病情稳定,≥5分为病情活动,将AS患者分为两组.分别比较患者组与正常对照绀,以及病情活动组与稳定组的IL-23R mRNA表达水平.结果 AS组的IL-23R mRNA表达水平(0.98±0.87)明显高于正常对照组(0.46±0.40),差异有统计学意义(P=0.0004);活动组表达水平(0.97±1.02)与稳定组(0.99±0.76)之间差异无统计学意义(P=0.2224).结论 IL-23R基因在强直性脊柱炎患者外周血高表达,可能是AS新的发病机制之一,进一步研究有可能会发现AS新的监测和治疗靶点.%Objective To investigate the gene expression of lnterleukin 23 Receptor( IL- 23R)in ankylosing spondylitis atients.Methods Total mRNA of peripheral blood white cells were obtained from 61 AS patients (satisfying the modified New York criteria,1984 )and 40 matched healthy subjects and their cDNA were synthesized. Disease activity was assessed by the criterion of the Bath ankylosing spon-dylitisdisease activity index(BASDAI) and defined into two levels:inactivity(average score ≤4)and activity( average score≥5 ). All the pa-tients received combined therapy of MTX+SASP+NASIDs . and none of them have been treated with biologics. Semi - Quantitative real -time PCR was used to determine the expression of IL -23R. Results The expression of IL -23R in AS patients were significant higher vs healthy controls (P=0.0004). Compared with inactivity patients (n=35 ), the expression of IL - 23 R in activity patients ( n = 26) shows no significant difference though it shows increase tendency (P = 0. 2224). Conclusion These results suggest that the expression of IL -23 R is up- regulated in ankylosing spondylitis and may contribute to AS pathogenesis and prognosis. IL- 23R may participate these immune disor-ders through it's essential function in IL- 23/Th17 axis which remains much more details to be further detected. Our findings suggest that the IL-23 pathway may also be a promising target for treatment of AS.
