Objective To investigate the value of ultra - early or early traditional craniotomy surgery for supratentorial large hypertensive intracerebral hemorrhage (HICH). Methods The clinical data of 17 cases of supratentorial large hypertensive intracerebral hemorrhage undertook ultra - early traditional craniotomy surgery were analyzed. Results 17 eases were all smooth operations. Hematoma was most removal in 7 cases and all removal in 10 cases. According to ADL ( ability daily life, ADL): grade I in 2 cases ( 11.8％ ) , grade Ⅱ in 4 cases (23.5％), grade Ⅲ in 6 cases (35.3％), grade IV in 3 cases ( 17.6％ ), grade V in 2 case ( 11.8％ ). Conclusion Supratentorial large hypertensive intracerebral hemorrhage undertaking ultra - early or early traditional craniotomy surgery reached definitely efficacy, mortality and morbidity was reduced.%目的 探讨早期超早期额颞大骨瓣开颅减压手术治疗幕上大量高血压脑出血的临床疗效及应用价值.方法 回顾性分析17例幕上大量高血压脑出血早期超早期行额颢大骨瓣开颅减压术的病例资料、手术疗效及随访结果.结果 17例患者均手术顺利,血肿全部清除10例,大部分清除7例.术后3个月,患者ADL(日常生活能力)分级:Ⅰ级2例(11.8%)、Ⅱ级4例(23.5%)、Ⅲ级6例(35.3%)、Ⅳ级3例(17.6%)、Ⅴ级2例(11.8%).结论 早期额颞大骨瓣开颅减压术治疗幕上大量高血压脑出血疗效确切,病死率和致残率低.
