骨髓侵犯致血细胞减少为首发表现的淋巴瘤患者临床特点分析

摘要

Objective to explore the laboratory and clinical characteristics of the lymphoma patients diagnosis as hematocytopenia.Methods Retrospectively reviewed 12 lymphoma patients in our hospital whose initial symptoms as hematocytopenia,and the related labora-tory examination and treatment information were analyzed.Results 3 patients diagnosed for T-NHL,9 patients diagnosed for B-NHL;8 were indolent lymphoma,most of the patients had different levels of splenomegaly,and ascended Lactate dehydrogenase level,part of the patients were negative for lymphadenectasis;ALL the patients were positive for flow cytometry analysis by bone marrow,while 6 patients were positive by bone marrow pathology detetion in 9 patients who were detected by both of the two methods.Most of patients recovered with the blood cells came to normal after the treatment.Conclusion Most of the lymphoma patients diagnosis for hematocytopenia as initial symptom were indo-lent B-cell subtype,who had long medical history and hard to diagnose;bone marrow biopsy cooperated with flow cytometry analysis could in-creased the positive rate of diagnosis.%目的：探讨骨髓侵犯致血细胞减少的淋巴瘤患者临床特点。方法选择芜湖市第二人民医院自2012年1月至2016年1月因收治的血细胞减少首诊的淋巴瘤患者12例，分析患者实验室检查和治疗情况。结果 T系淋巴瘤3例，B系淋巴瘤9例，其中惰性淋巴瘤8例。患者表现为不同程度脾脏肿大、乳酸脱氢酶升高等，部分患者无淋巴结肿大。9例患者同时行骨髓活检和流式免疫分型，流式检测均阳性，骨髓活检6例呈阳性。经抗淋巴瘤治疗后，血细胞不同程度的恢复。结论血细胞减少的淋巴瘤患者以惰性淋巴瘤多见，骨髓活检联合流式免疫分型检查可提高诊断阳性率。
