Objective To explore the effect of cluster-based nursing strategy on the treatment of postoperative delirium ( POD) in the elderly patients with hip fracture. Methods 63 cases of elderly patients with hip fracture, underwent operation in our hospital from Jun 2014 to Jun 2016, were divided into the treatment group (32 cases) and the control group (31 cases) by simple randomization method. Be-sides the routine nursing service for patients in the two groups, the patients in the treatment group were further given the cluster-based nurs-ing treatment. The incidence of POD, sleep quality index, pain score and average hospital stays between the two groups were compared. Re-sults The incidence of POD in the treatment group was 3. 12%, which was significantly different from the incidence of 22. 58% in the con-trol group (P<0. 05). The average hospital stays, sleep quality index and pain score in the treatment group were (16. 82 ± 2. 82) days, (9. 23 ± 2. 15) and (3. 87 ± 1. 87), respectively, whereas in the control group they were (18. 61 ± 3. 72) days, (11. 24 ± 4. 31) and (5. 32 ± 1. 51), respectively, and significant differences were found between the two groups (all P<0. 05). Conclusion The perioperative treat-ment with cluster-based nursing strategy in the elderly patients with hip fracture could effectively reduce the incidence of POD, alleviate their pain and improve their sleep quality, and further shorten the average hospital stays and improve the postoperative quality of life.%目的 探讨集束化护理策略在预防老年患者髋部骨折术后谵妄(POD)的应用效果.方法 选取2014年6月至2016年6月在安徽医科大学第四附属医院行手术治疗的老年髋部骨折患者63例,采用简单随机化方法将病例分为对照组31例及干预组32例.对照组采用常规护理方法,干预组在对照组基础上采用集束化护理干预,比较两组患者POD发生率、睡眠指数及疼痛、平均住院日.结果 干预组患者POD发生率为3.12%,对照组为22.58%,两组比较差异有统计学意义(P<0.05),干预组平均住院日、睡眠指数及疼痛评分分别为(16.82±2.82)d、(9.23±2.15)分和(3.87±1.87)分,对照组分别为(18.61±3.72)d、(11.24±4.31)分和(5.32±1.51)分,两组比较差异均有统计学意义(P<0.05).结论 老年髋部骨折患者围手术期实行集束化护理管理,可有效降低患者POD发生率、减轻疼痛、改善睡眠质量,缩短了平均住院日,显著提高患者术后生活质量.
