肝细胞癌中磷酸化STAT3与上皮型钙粘素的表达及其临床意义

摘要

目的 探讨信号转导和转录激活因子3(STAT3)活化形式--磷酸化STAT3(p-STAT3)和上皮型钙粘素(E-cadherin)在肝细胞癌中的表达、相互关系及其临床预后意义.方法 免疫组化法检测100例临床肝癌标本及10例正常肝组织标本中的p-STAT3和E-cadherin的表达,分析其与肝癌临床病理特征的相关性;以Kaplan-Meier生存分析及Logrank检验其表达水平与肝癌患者总体生存的关系.结果 p-STAT3在肝癌中呈高表达(58%),与肿瘤侵袭转移密切相关(P=0.038);52%肝癌组织中存在E-cadherin表达降低,与肿瘤侵袭转移亦存在明显相关性(P=0.001);在肝癌组织中p-STAT3与E-cadherin表达呈显著负相关(P=0.006).生存分析提示p-STAT3阳性表达、E-cadherin降低表达预示肝癌患者不良预后(P=0.005,P=0.001).结论 STAT3信号通路活化、E-cadherin异常表达与肝癌侵袭转移密切相关,p-STAT3及E-cadherin有望作为肝癌患者预后判断的参考指标.%Objective To investigate the expression of activated phosphorylated signal transducer and activator of transcription 3 ( p-STAT3 ) and E-cadherin in hepatocellular carcinoma( HCC), and to analyze their correlations and predictive effects on HCC prognosis. Methods The expression profiles of p-STAT3 and E-cadherin were detected in 100 HCC clinical samples and 10 normal liver tissues by immunohistochemical staining method, and their correlations with clinicopathological parameters were also analyzed. The association between p-STAT3, E-cadherin expression and prognosis was assessed by Kaplan-Meier survival analysis and compared by log-rank test. Results The results demonstrated that p-STAT3 expression was positive in 58％ HCC patients. The expression of p-STAT3 was associated with invasion and metastasis( P = 0. 038 ), However,52％ HCC showed that E-cadherin expression reduced,it was also associated closely with invasion and metastasis ( P = 0. 001 ). In HCC, p-STAT3 expression was negatively correlated with E-cadherin expression( P = 0. 006). Survival analysis indicated that HCC patients with pSTAT3 positive expression or reduced E-cadherin expression had a significantly shorter survival duration than those with negative expression of p-STAT3 or those with normal E-cadherin expression( P = 0. 005, P = 0. 001 ). Conclusion Activated STAT3 signal and deregulated E-cadherin expression are correlated with HCC invasion and metastasis. Abnormal p-STAT3 or E-eadherin expression can serve as markers for prognosis judgment of HCC patients.
