低摩擦轻力矫治器与MBT矫治器拔牙矫治的牙齿移动分析

         

摘要

目的 比较低摩擦轻力矫治器与MBT直丝弓矫治器在拔牙矫治过程中牙齿移动的特点,为临床应用提供参考.方法 选择双牙弓前突患者40例,平均分为两组,一组采用低摩擦轻力矫治器,不采用支抗增强措施,另一组采用MBT矫治器,采用口外弓支抗增强的措施.治疗前后对患者拍摄X线头颅定位侧位片,定点、测量19项测量项目,并对测量结果进行两组独立样本的t检验.结果 两组X线头影测量:骨性指标∠SNA、∠SNB、∠ANB 、∠MP-FH治疗前后本组间、治疗后两组间均没有显著变化,差异无统计学意义.牙性指标∠UI-SN、∠UI-PP、∠L1-MP、∠U1-L1、UIA-PTV、UIE-PTV、LIE-PTV、LIA-PTV、UM-PTV、LM-PTV治疗前后本组间均有显著变化,差异有统计学意义(P<0.01),治疗后两组间没有显著变化,差异无统计学意义.软组织指标UL-EP、LL-EP、Z角治疗前后本组间均有显著变化,差异有统计学意义(P<0.01),治疗后两组间没有显著变化,差异无统计学意义.结论 应用低摩擦轻力矫治器治疗双牙弓前突的患者,在没有采取增强支抗措施的条件下,仍可以得到较好的支抗控制及前牙转矩控制,上下前牙内收明显,软组织侧貌改善明显.%Objective To explore the character of tooth movement in the extraction cases between light force orthodontic appliance and MBT straight wire appliance so as to provide references for clinical applications. Methods 40 patients with dual-arch protrusion were chosen, and divided into two groups. In one group, the patients were treated using light force orthodontic appliance without any measures to enhance anchorage. In the other group, the patients were treated using MBT straight wire appliance with headgear anchorage. The X-ray cephalometry films were taken before and after treatment, and 19 items were chosen as evaluating indicators. The results were compared by two independent t tests. Results There was no significant difference on the bone index ( ∠SNA, ∠SNB, ∠ANB, ∠MP-FH ) in the intra-group before and after treatment and in the inter-group after treatment. There was significant difference on the tooth index( ∠UI-SN, ∠UI-PP, ∠L1-MP, ∠U1-L1 , UIA-PTV, UIE-PTV, LIE-PTV, LIA-PTV,UM-PTV ,LM-PTV ) in the intra-group before and after treatment,but no significant difference in the inter-group after treatment. There was significant difference on the soft tissue index( UL-EP, LL-EP, angle Z ) in the intra-group before and after treatment, but no significant difference in the inter-group after treatment. Conclusion Without any measures to enhance anchorage, the patients with dual-arch protrusion can be treated well using low friction and light force appliance. The control of anchorage and anterior teeths torque is well achieved. The changes of incisor retraction and soft tissue profile are both obvious.
