冲击波结合便携式热敏肩部理疗仪治疗肱二头肌长头肌腱炎

摘要

目的 观察冲击波结合便携式热敏肩部理疗仪治疗肱二头肌长头肌腱炎的临床疗效.方法 2014年6月-2016年6月本院收治的肱二头肌长头肌腱炎患者58例,年龄23.0～ 64.0(49.0±5.8)岁,随机分为治疗组和对照组各29例.治疗组采用冲击波结合便携式热敏肩部理疗仪温热治疗,对照组采用冲击波结合红外线灯照射温热治疗.对比两组治疗前和治疗后1个月、3个月的视觉模拟评分(visual analogue scale,VAS)及Constant-Murley肩关节功能评分(constant-murley score,CMS).结果 所有患者进行至少3个月的随访,治疗过程中无明显并发症.两组治疗后VAS评分及CMS评分较治疗前均有明显改善.治疗后1个月随访时,治疗组VAS评分为2.8±1.6,对照组为4.2±1.3,差异有统计学意义(P＜0.05);治疗组CMS评分为82.3±8.6,对照组为72.7±8.8,差异有统计学意义(P＜0.05).治疗后3个月,治疗组VAS评分为1.5±1.9,对照组为2.3±1.6,差异无统计学意义(P=0.185);治疗组CMS评分为93.4±9.2,对照组为81.8±10.1,差异有统计学意义(P＜0.05).结论 冲击波结合便携式热敏肩部理疗仪治疗肱二头肌长头肌腱炎在早期即可较好地缓解疼痛,促进肩关节后期功能恢复.%Objective To summarize the clinical outcomes of shock wave combined with portable thermal shoulder physiotherapy for treatment to tendinitis of long head of biceps.Methods From June 2014 to June 2016,58 patients with tendinitis of long head of biceps were treated in our hospital,with an average age of (49.0 ± 5.8) years,and they were randomly divided into treatment group (n=29) and control group (n=29).Patients in treatment group were treated by shockwave combined with portable thermal shoulder physiotherapy,and the control group by shockwave combined with infrared lamp treatment.Comparation was conducted before treatment and at 1 month and 3 months after treatment.Results All patients were followed up for at least 3 months,and there was no complication during the treatment.The visual analogue scale (VAS) and constant-murley score (CMS) improved significantly after treatment in both two groups.At 1 month after treatment,VAS of treatment group was 2.8 ± 1.6 and control group was 4.2 ± 1.3 (P ＜ 0.05),CMS of treatment group was 82.3 ± 8.6 and control group was 72.7 ± 8.8 (P ＜ 0.05),with statistically significant difference between two groups.At 3 months after treatment,VAS of treatment group was 1.5 ± 1.9 and control group was 2.3 ± 1.6with no significant difference (P=0.185).CMS of treatment group was 93.4 ± 9.2 and control group was 81.8 ± 10.1,with statistically difference between two groups (P ＜ 0.05).Conclusion Shock wave combined with portable thermal shoulder physiotherapy can effectively relieve pain at early stage,as well as improve shoulder joint function recovery in late stage in patients with tendinitis of long head of biceps.
