窒息足月新生儿血乳酸值及血糖浓度的短期变化

摘要

目的：探讨窒息足月新生儿血乳酸水平与血糖浓度的短期改变及其临床意义。 方法：159例足月窒息新生儿根据Apgar评分分为轻度窒息组、重度窒息组和Apgar评分正常的窒息组，20名健康足月新生儿为健康对照组，在出生后1小时、6小时、24小时分别采集动脉血。用I-STAT微量血气分析仪和快速血糖分析仪测定血乳酸和血糖水平。比较各组问数据的差异及相关性。所有数据用SPSS13.0处理。 结果： (1)出生后1小时、6小时和24小时三个时间点的血糖水平：健康对照组分别为(4.99±1.14)、(4.35±1.98)和(5.12±1.55)mmol/L；轻度窒息组分别为(4.39±1.95)、(4.60±1.26)和(4.73±1.53)mmol/L；重度窒息组分别为(4.51±2.00)、(4.96±1.89)和(4.72±1.38)；Apgar评分正常的窒息组分别为(3.40±1.40)、(4.40±1.22)和(4.57±1.38)mmol/L。 (2)出生后1小时、6小时和24小时三个时间点的血乳酸水平：健康对照组分别为(3.51±2.43)、(3.07±2.29)和(4.19±2.88)mmol/L；轻度窒息组分别为(5.38±2.81)、(3.80±2.20)和(4.49±2.45)mmol/L；重度窒息组分别为(9.17±4.93)、(5.21±3.06)和(5.55±3.04)mmol/L；Apgar评分正常的窒息组分别为(3.27±1.82)、(3.79±2.26)和(4.18±2.53)。 (3)在出生后1小时时间点，血乳酸水平与Apgar评分呈负性对应关系(R2=0.454，p<0.01)，与血糖水平呈正性对应关系(R2=0.035，P<0.05)。 结论：血糖水平对于评估足月新生儿窒息严重程度的临床意义不明显。足月窒息新生儿出生后短时间内，血乳酸水平与血糖水平之间存在一定的正性相关性。血乳酸可帮助评估窒息严重程度，可作为临床诊治足月新生儿窒息的辅助实验室指标。
