The study on marine accidents has been a keen subject in ocean engineering for long. This paper develops an HFACS-EI Framework based on the original HFACS Framework. The framework is further combined with Cognitive Map (CM) in marine accident evolution analysis which is divided into four stages. A hybrid accident analysis approach combining HFACS-EI Framework and CM not only provides classification of human factors under six levels, but also judges if there is a positive relation between every two human factors. This paper further analyzes the positive relations between human factors at different accident evolution stages and specifies what human factors are of more significance in a certain accident phase. A serious marine accident of capsized ship is chosen as a typical case to illustrate how the hybrid approach is applied in practice. Consequently, the proposed method can effectively identify and analyze human factors under six levels at different accident evolution stages and help to reduce human errors in marine accidents.
