I believe that if the necessary, high-quality management systems for each of the top ten FEMI issues covered in this paper were in place and sustained over the long haul by knowledgeable FEMI specialists and inspectors at each operating site, then the vast majority of process safety incidents caused by FEMI failures would go away. Achieving excellence in your FEMI program is not rocket science and does not take the skill level of a neurosurgeon. As in the sport of football, it's fairly basic blocking and tackling. Certainly there is a lot more to this story than just the brief summary presented in this paper. For more understanding and enlightenment of FEMI issues like those above, the bi-monthly issue of the Inspectioneering Journal is a good source of articles on all sorts of FEMI subjects. Additionally, the biannual API Inspection Summit is another excellent source of the latest information pertinent to FEMI jobs. Notice that in this paper I did not include many FEMI issues in my top ten like pressure vessel inspection, pressure relief valve inspection and servicing, welding inspection, QA/QC of repairs, heat treatment, regulatory compliance, and handling equipment overdue for inspection just to name a few. These are some of the basic foundational FEMI issues out of the 101 FEMI issues that most sites handle fairly well. The focus of this paper is on those FEMI issues that are not always handled as well as they need to be and therefore are included in the Top Ten Reasons for FEMI Failures in the Hydrocarbon Process Industry.
