The human error is a major cause in industrial accidents and the proportion of human error has the tendency to rise gradually. The goal of this study is to discover the construction and factors of human reliability and un-safety behaviors. The methods were questionnaire analysis and error analysis. First, to investigate the operation of tanker with Allyl Alcohol and the operation of pouring Vinyl Acetate in a chemical factory. The error-identify method and real site evaluation were coordinated with each other to evaluate the module of human error in the chemical factory. The result of this research contributed to the relative research in quantization of the human error and help to prevent the human error from happen.
