The purpose of research is to identify the causes of defects in the cut to box second building division of PT Parkland World Indonesia, calculate the percentage of defects that occur in the cut to box division of the second building division of PT Parkland World, and provide solutions to repair the defects that occur. This study uses Statistical Process Control (SPC) and Failure Mode Effect Analysis (FMEA) methods. Based on the results of data processing found in the shoe production process there are 10 types of characteristics that cause defects that become the Critical to Quality (CTQ) key, namely Bonding (21.6%) and then followed by Gross (16.3%). Broken Stitches (15.7%). Over cementing (10.4%) Heel height (10.2), Under / over Roughing (9.9) Sole Long / Short (8.5), Backstay Slanted (4.2), Inaccurate position / Shake (2.2), the smallest is Leaning Lasting (1.1%). CTQ results with a histogram diagram show that Bonding is the highest type of defect with a percentage of 21.6%, followed by Gross with a percentage of 16.3% and a broken stitch percentage of 15.7%. Improvement solutions by evaluating the performance of operators in each production process, conducting an internal audit of the production process, checking the oven machine regularly, conducting an internal audit of the work process and the use of glue and glue composition. Conduct 6S audit in the work area in order to achieve the specified quality standards.
