Applications of Cause-Consequence Diagrams in Operational Risk Assessment

Cause-Consequence Analysis (CCA) is emerging to be a very useful tool to depict and maintain an up-to date, real-time working risk management system embedded in daily operations. Bowtie diagrams (bowties) are a less formal CCA that provide a pictorial representation of the risk assessment process [1]. The authors believe that bowties are a resourceful tool to improve performance of the hazard identification and risk assessment process (HIRA) in process industries, to demonstrate that major hazards are identified and managed to as low as reasonably practicable (ALARP).
