POSITIVE FLOW CYTOMETRY CROSSHATCH DUE TO LOW LEVELS OF Bw6 ANTIBODIES DETECTED BY LABScreen~R ASSAY

This case report focuses on a sensitized kidney transplant candidate with weak reactivity against several HLA-B locus antibodies as determined with LABScreen~R. Each reactive bead expressed the HLA-Bw6 epitope. Although the detected antibodies are below our positive cut-off threshold, it led to a strong positive crossmatch. This case report suggests that the LABScreen~R assay can detect HLA antibodies generally considered too weak to result in a positive crossmatch and careful consideration should be given to the pattern of reactivity of beads. Signal strength alone is not a sufficient metric to assess the potential of a so-called weak antibody to result in a positive corssmatch.
