The flexor tendon sheath ganglion develops mainly from the Al or A2 pulley, which makes up 10-12% of ganglions in hand and wrist12.This ganglion is observed as small nodules on the palmar area in metacarpophalangeal joint, and causes tenderness and pain in gripping fingers. However, it has been reported that this ganglion rarely causes symptoms of a trigger finger3. We encountered a case of a flexor tendon sheath ganglion on the Al pulley that acted as a cause of a trigger thumb.
