A Case Report of Salter Harris IV Injury of Distal Radius

Epiphyseal injuries represent between 18.5 & 30 %u2 of pediatric fractures. Most of these injuries are Salter Harris II injuries. Salter Harris IV injuries are infrequent and tend to involve proximal tibial epiphyses.JH, a 15 year old boy sustained injury to right dominant wrist when he landed onto his outstretched hand from a height of 3 ft. He presented the next day with a tender swelling on the dorsum of the wrist. It was an isolated closed injury without neurovascular compromise. Radiographs revealed a Salter Harris (SH) IV fracture of the dorsal aspect of the radius with 2-3 mm displacement (Figure 1,2). The fracture was reduced and fixed with a Kirchner wire in an acceptable position (Figure 3,4). The K-wire was removed 4 weeks later. He has had 18 months of follow up and has good pain free range of motion not affecting his daily activities.
