Treatment of distal radius fractures has been advanced a great deal during the last decade and continues to evolve. Volar locking plates and release of brachioradialis improved the final results.While the author believes the volar approach has many advantages over other surgical options, he also encountered some issues to overcome, such as troubles dealing with dorsal comminution, less than ideal screw placement, less than ideal fixation to allow early motion, problems with radial translation of the distal fragment, and less than ideal reduction of the fracture especially when there are many small fragments.
