A New Distraction Treatment of Mandibular Condylar Fractures: Application of a Dynamic Internal Distraction Device

To improve unfavorable results of closed treatment and open reduction of mandibular condylar fractures, we have developed a new treatment applying a dynamic internal distraction device which allows for simultaneous stabilized protection of the TMJ and fracture locus during healing process, and maintenance of a smooth motion to prevent the TMJ dysfunction. Between 2000 and 2005, we had performed this treatment on ten cases in satisfactory results This treatment is low risk and the minimally invasive method of choice for surgical management of mandibular condylar fractures, especially intracapsular and high level neck fractures with difficulty in rigid fixation. This treatment is not anatomically reduction-oriented but functionally recovery-oriented because of allowing for the TMJ movement in early postoperative phase.
