NATHCARE - Networking Alpine Health for Continuity of Care. The pilot case in the Province of Trento

Mountain areas in the Alps present specific logistic problems, but the vitality and the resources of the local communities offer also many opportunities for development and requalification. In the context of the Alpine Space Programme 2007-2013, the EU transnational co-operation programme for the Alps, the NATHCARE project aims to develop an organizational model of shared and integrated care and an IT platform able to support it. In this paper, we present both the project and the NATHCARE platform, an IT tool to support and improve the care process model, and their application in a pilot case in Province of Trento.
