Product integrity through out a products life can not be left to chance, a systematic process is essential. This process must combine prediction based on analysis of history together with analysis of failures that could possibly happen but haven't yet occurred. The process has to cover all aspects including human factors and be focused on the consequence as well as the likelihood. The Non-Advocate process, using both BMT Reliability Consultants and independent internal engineers to bring fresh insights into the study, greatly enhanced the review. This process we believe achieved our stated objectives and produced a prioritise risk assessment with a recommended action plan. The aggressive implementation of mitigation activity by BA and RR inconjunction with the other partners is providing significant additional assurance for the engine aspects of Concorde operation, an operation which was already safe by the defined standards.
