The electronic medical record (EMR) system becomes popular in Japan. However, the exchange of digital patient data among the medical institutions is still rather limited. 1. Methods: Since most EMR systems in Japan have some kinds of patient profile information (PPI) to survey patient clinical characteristics, we investigated the common items of the PPI among the EMR systems and of the medical questionnaires for new patients.
