Extracapsular Proximal Femoral Fractures: Indications for (Primary) Arthroplasty

Worldwide, osteosynthesis of extracapsular proximal femur fractures is routinely performed. Meanwhile, in cases of osteoporosis and/or comminuted fractures several complications can occur in old patients, leading to revision to an arthroplasty. Primary arthroplasty can minimize these complications allowing the patient to walk immediately with full weight bearing. The implantation of the prosthesis is not demanding provided the operation is planned accurately using templates and full size radiographs and provided one uses the correct surgical approach. The aim of this article is to present the operative technique, the results, the rationale for this indication and to discuss this point of view in relation to the literature.
