Background: lesions of the fifth cranial nerve cause a sensory deficit to the affected side of the face. They may be caused by oncologic resection, trauma and iatrogenic lesions such as third molar extractions and orthognathic surgery.Methods: with the introduction of supermicrosurgery it is now possible to perform surgery singular fascicular nerve. In this paper Authors present their application of two different supermicrosurgical techniques for third trigeminal branch palsy.Results: post - operative trigeminal reflex and evoked potential tests show functional recovery of the region.Conclusion: Surgical treatment of the third trigeminal branch is still challenging. The introduction of supramicrosurgical techniques allows single nerve fascicles anastomosis, guaranteeing a fast sensitivity recovery with the lowest surgical stress for the patients.
