Rapid Determination of Tramadol and N-Desmethyltramadol in Human Urine by HPLC-MS-MS Optimized Method

A 1200 series LC pump coupled to a 6460 triple-quadrupole MS operating in positive ESI mode was used for analysis. The best results were obtained by using a mobile phase that consisted of 0.01% formic acid and 5mM ammonium formate in water (Solvent A) and 0.01% formic acid in methanol (Solvent B)The mobile phase composition was 90% A & 10% B at a flow rate of 0.6 mL/min. The optimum results were achieved by using: Poroshell 120 EC-C18, 2.1*50mm,2.7-Micron column A total of 14672 samples were successfully tested at PACTOX by this method in 2012. Of these, 8947 were from patients who had been prescribed Tramadol. Of these patients, 2362 of the samples were positive and 6585 were negative. We also discovered that 5723 samples were positive even though the medication had not been prescribed, or provided as a prescribed medication by the physician. Based on this data, 26% of the patients who were prescribed Tramadol were positive and therefore taking their medication. 74% of the patients who prescribed Tramadol were not taking their medication. 40% of the total patients tested positive for Tramadol who had not been prescribed the medication.
