87-lead BSPM compared activation duration in RV, septum and LV of 28 patients, mean 60 years, HF in NYHA class II-IV, EF 27.8, LBBB QRS 180.2 ms, and control group, in(1)normal; (2) LBBB and RV single-site pacing; (3) LBBB without pacing; (4) Biventricular pacing, aiming to improve selection for BP implantation. The longest activation time was in (2), with RV duration (87.5 ms) longer than (I and (3) (35.2 ms and 42.7 ms, pO.Ql). Septal duration was approximately the same as without pacing, both in normal and LBBB individuals (around 50 ms), however duration increased under(2) and (4). LV duration decreased in (4) (LBBB - 84.9 ms to Biv - 72.9 Ms, p<0.01). RV duration increased in (4), compared to (1) (35.2 ms to 71.8 ms, p<0.01), and decreased compared to (2) (87.5 ms to 71.8 ms, p<0.01) (Figure 1). RV impairment in LBBB might explain "unresponsiveness" to BP.
