This contribution introduces a procedure for volume-controlled non-invasive mechanical ventilation under consideration of leakages and spontaneous breathing. The identification and flow feedback control implemented were tested with a nonlinear simulation model of a single-hose home-care ventilator with sensors inside the device. Measured values of clinical tests were used to examine the influence of spontaneous breathing on the identification. The suggested method permits a compensation of the leakage and at the same time a classification of the breathing effort of the mechanically ventilated patients for the first time. Since only data of the inspiration interval is needed, the suggested procedure applies even to simple and economical ventilators common in home care. As a consequence sophisticated monitoring, adaptation of the mechanical ventilation level according to the patient's needs and better acceptance by the patient are possible, since the procedure is accomplished without interrupting the routine mechanical ventilation.
