This paper describes a model to reduce the severity of the hazards associated with striking underground gas pipelines in the excavation phase of the construction process. The model emphasizes the different kind of accidents, their causes, and errors that lead to these accidents. A fault tree is developed to establish a causation model and minimal cut sets. An advisory system was also developed to facilitate the risk evaluation in the excavation activity, and to suggest preventative measures that should be considered. An example is given to illustrate the use of the fault tree analysis and the implication of this model.
