Impingement in ceramic hip patient a retrieval and wear-scar analysis of femoral head

In the rigid-rigid THR, the incidence of impingement, subluxation and dislocation have been of some interest. In this case of all-ceramic THR, we followed a patient who presented with a loud clicking noise as she ambulated. At retrieval operation, the posterior femoral neck had two notches. The protruding rim of the metal cup was excised with a high-speed burr. SEM analysis showed that main wear zone (MWZ) had high wear, and peripheral wear zone (PWZ) had severe wear. Therefore these data indicade that poor positioning of a rigid socket may not only create the problems of impingement but may also accelerate the wear of the ceramic ball.
