The genetic basis of inter-individual differences in the ability to develop collateral vessels in patients with chronic coronary artery disease (CAD) is unknown. We examined whether functional single-nucleotide polymorphisms (SNPs) in genes involved in arteriogenesis and angiogenesis were associated with the development of collateral coronary circulation (CCC) in those patients. A total of 459 consecutive patients with severe chronic coronary artery stenoses were included. The C allele of C-604T KDR was associated with well developed CCC in patients with > 70% (n=240, p 0.012) or > 95% (n=91, p 0.011) chronic single vessel coronary stenoses. This association was not influenced by several clinical covariates analysed. This allele is associated with a lower transcriptional activity and reduced plasma soluble KDR levels, which may have functional consequences on VEGF signalling pathway.
